১. হাসপাতালের চিঠি সম্পূর্ণ নয়
চিঠিতে রোগীর পাসপোর্ট অনুযায়ী নাম, জাতীয়তা, পাসপোর্ট নম্বর, চিকিৎসার বিভাগ, ডাক্তার/কোঅর্ডিনেটর, সম্ভাব্য ভর্তি বা অ্যাপয়েন্টমেন্ট তারিখ এবং হাসপাতালের যোগাযোগ থাকতে হবে।
২. নাম বা পাসপোর্ট নম্বর মেলে না
পাসপোর্ট, আবেদন ফর্ম এবং হাসপাতালের চিঠিতে নামের বানান ও পাসপোর্ট নম্বর একই হতে হবে। ছোট ভুলও দেরি করতে পারে।
৩. অ্যাটেনডেন্ট তথ্য পরিষ্কার নয়
স্বামী/স্ত্রী, বাবা-মা, সন্তান বা আত্মীয় গেলে রোগীর সঙ্গে সম্পর্ক এবং রোগীর তথ্য স্পষ্টভাবে দিতে হবে।
